What Is Vasopressin (ADH)?

Vasopressin, also known as antidiuretic hormone (ADH), is a small peptide hormone produced in the hypothalamus and released by the posterior pituitary gland. Its primary job is to help your body regulate water balance. When your body senses that blood is becoming too concentrated or blood volume is dropping, ADH signals the kidneys to hold onto water rather than excrete it in urine.

Beyond water conservation, vasopressin also plays a role in constricting blood vessels, which can influence blood pressure. Because ADH sits at the intersection of hydration, kidney function, and cardiovascular regulation, measuring it can offer clues about a range of conditions when your fluid balance appears disrupted.

What the Test Measures

The vasopressin (ADH) test measures the concentration of antidiuretic hormone circulating in your blood. This value is most meaningful when interpreted alongside other measurements, particularly:

  • Serum sodium and osmolality — how concentrated your blood is
  • Urine osmolality and volume — how concentrated or dilute your urine is
  • Fluid intake and hydration status at the time of the draw

Because ADH normally rises and falls in response to your hydration state, a single value in isolation can be difficult to interpret. Your clinician typically looks at whether your ADH level is appropriate for your blood concentration at that moment.

What High Levels Can Mean

Elevated ADH means your body is signaling the kidneys to retain more water than expected. This can dilute your blood and lower sodium levels. Higher-than-expected vasopressin levels may be associated with:

  • Syndrome of inappropriate antidiuretic hormone secretion (SIADH), in which ADH is released even when it is not needed
  • Dehydration or reduced blood volume, where elevated ADH is an appropriate response
  • Certain lung conditions or central nervous system disorders
  • Some medications that stimulate ADH release
  • Certain tumors that can produce ADH-like substances

Because high ADH can lead to low blood sodium (hyponatremia), symptoms may include headache, nausea, confusion, or fatigue. These findings require professional interpretation to identify the underlying cause.

What Low Levels Can Mean

Low or inappropriately low ADH means the kidneys are not receiving the signal to conserve water, which can lead to excessive urination and increased thirst. Reduced vasopressin activity may be associated with:

  • Central diabetes insipidus, in which the brain does not produce or release enough ADH
  • Excessive fluid intake (primary polydipsia)
  • Damage to the hypothalamus or pituitary gland from injury, surgery, or other causes

A related condition, nephrogenic diabetes insipidus, involves normal or high ADH levels but kidneys that do not respond to the hormone. Distinguishing between these possibilities often requires additional testing, such as a water deprivation study, guided by your clinician.

How the Test Is Done

Vasopressin is measured through a blood sample, typically drawn from a vein in your arm. Because ADH is a relatively fragile molecule, the sample often needs to be handled carefully, chilled, and processed promptly, so it is usually collected in a clinical setting.

Your clinician may ask you to follow specific preparation instructions, which can include guidance about fasting, hydration, or certain medications. In many cases, the ADH test is performed together with sodium and osmolality measurements, or as part of a supervised water deprivation test, so that your result can be interpreted in the proper physiological context.
